2. Declaration to certify the conjugal(marriage) relation a. In case of losing the certificate of marriage, the declarant to apply for reissue of the documents to certify the conjugal relation by the civil branches of government shall submit the following materials:

Photocopy of the declarant’s Identity Card
(2).
Original of certificate of Absence of Marriage Record issued by Hong Kong Marriage Registry
Optional to provide:
(1)．
Photocopy of his/her spouse’s Identity Card
(2)．
Photocopy/photocopies of the other documents to certify their conjugal relation
b. If the declarant, who had been married before the Law of Marriage was enacted in 1950, applies for notarization of marriage, the following materials shall be submitted:
(1)．Photocopy of the declarant’s Identity Card
(2).
Original of certificate of Absence of Marriage Record issued by Hong Kong Marriage Registry
Optional to provide:
(1)．Photocopy of his/her spouse’s Identity Card
(2)．Photocopy/photocopies of the other documents to certify their conjugal relation
3. Declaration to claim or disclaim his/her succession a.
To claim the succession, the following materials shall be submitted:
(1)．
Photocopy of the declarant’s Identity Card
(2)．
Photocopy of the death certificate of the ancestor, who died in HongKong
(3)．
In case of testate succession, photocopy of the will of the ancestor is required.
Optional to provide:
(1)．
Photocopy of the documents to certify the kinship between the declarant and his/her ancestor
(2)．
Photocopy of the death certificates of the ancestor (died out of HongKong) and/or, if any, other inheritor(s)
(3)．
Photocopy of the certificate of inheritance
b.
To disclaim the succession, the following materials shall be submitted:

Photocopy of the declarant’s Identity Card

Photocopy of the documents to certify the kinship between the declarant and his/her ancestor
4. Declaration to apply for entry to HongKong for Mainland relatives a.
To apply for Mainland spouse’s residence in HongKong, the following materials shall be submitted:
(1)．
Photocopy of the declarant’s Identity Card
(2)．
Photocopy of the declarant’s certificate of marriage
(3)．
Original of Certificate of Absence of Marriage Record issued by Hong Kong Marriage Registry, or Application for Unmarried Record
(4)．
If there are any children aged under 14 to apply for entry to HongKong together, their certificates of birth are required.
Optional to provide:
(1)．
Original or confirmation or photocopy of any other certificates related to application
b.
To apply for other Mainland relatives’ residence in HongKong, the following materials shall be submitted:
(1)．
Photocopy of the declarant’s Identity Card
(2)．
Photocopy of the documents to certify the kinship between the declarant and the applicant

According to the requirement of the competent authorities in Mainland China, when the declarant applies for children’ residence in HongKong, the photocopy of the certificate of no children in Hong Kong may need to be provided; if the declarant’s children have immigrated to oversea, the photocopy of the certificate of living abroad may need to be provided. |

5. Declaration to apply for Mainland relatives’ entry to Hong Kong to take over property(ies), the following materials shall be submitted: (1)．

Optional to provide:

Original or confirmation or photocopy of any other certificates related to application
6. Declaration to apply to adopt a child from Mainland China a.
If applicants are a couple, the following materials shall be submitted:
(1)．
Identity Card of the declarant
(2)．
Original of application form for adoption
(3)．
Original of guarantee
(4)．
Photocopy of the certificate of marriage
(5)．
Original or photocopy of the documents to certify the declarant’s occupation and income
(6)．
Original or photocopy of the certificate of the declarant’s health record issued by a hospital or a certified doctor
Optional to provide:
(1)．
Confirmation of the certificate of (no) criminal conviction of the declarant
b.
If applicant is a bachelor/spinster, the following materials shall be submitted:

Original of application form for adoption
(3)．
Original of guarantee
(4)． Original of Certificate of Absence of Marriage Record issued by Hong Kong Marriage Registry, or
Photocopy of the divorce certificate, or

Original or photocopy of the certificate of the declarant’s health record issued by a hospital or a certified doctor

If applicants are a couple, the following materials shall be submitted:

Photocopy of the certificate of marriage
(3).
Photocopy of the Mainland Identity Card of the adopted child
Optional to provide:
(1).Photocopy of the related documents to certify the adoption (such as: agreement of adoption, correspondence, evidence of remittance, evidence of supply, etc.)

If applicant is a bachelor/spinster, the following materials shall be submitted:
(1).
Photocopy of the declarant’s Identity Card
(2). Original of Certificate of Absence of Marriage Record issued by Hong Kong Marriage Registry, or Photocopy of the divorce certificate, or Photocopy of the death certificate of the declarant’s spouse

Photocopy of the Mainland Identity Card of the adopted child

8. Declaration to guarantee for a Mainlander resident to study aboard, The following materials shall be submitted: (1).
